PhoneUnite is a US-based Business Provider, HQ in Kansas City Missouri but also with offices in Vancouver, BC.
Similar to other VoIP Business providers no contract is required and the most basic plan starts from $9.95 w/1-line for 3-users to $169.95 w/10-lines unlimited users with all the VOIP features you can expect including smartphone app (only for iPhone), though note the minutes included in the plans tend to be in the lower end more so considering these are shared among extensions - though rates are otherwise inexpensive. Free trial available. May also look at Versature and/or RingCentral since the plans are better and more comprehensive with a lower entry monthly price-tag.
